Prairie du Chien Memorial Hospital, located at 705 East Taylor Street, is a 25 bed Critical Access Hospital licensed by the State of Wisconsin and accredited by the Joint Commission on Accreditation of Health Care Organizations. The hospital health care team includes nearly 300 staff members.  The Medical Staff has 16 active staff with 55 consulting specialists and allied health professionals.  Services provided to area residents by the hospital organization numbers over 60.  These services range from the traditional acute care (medical, surgical, obstetrics, pediatrics, ICCU) skilled, and intermediate care to a wide variety of outpatient programs (audiology, sleep study, foot care, rehab therapy services, ophthalmology, respiratory therapy and diagnostics), Home Health, Hospice, Telemonitoring, Assisted Senior Living, Family Resource Center, Child Day Care, Childbirth Education, Adult Day Care rounds out the home and family programs.  Prevention services include congregate meals, meals on wheels, health screenings, parish nursing, C.A.R.E.  Crawford Abuse Resistance Effort, wellness education.  The Emergency Department staffs 24/7  with physician, nursing and diagnostic services team who care for emergency and urgent care needs.  An expanded list of outpatient and community services is available on the hospital website or call 608-357-2000.

The Gundersen Lutheran – Prairie du Chien Clinic is located at 610 East Taylor Street. It provides the right care for the entire family. A team of five family practice physicians, one internal medicine physician, two physician assistants, two surgeons, four nurse practitioners, a nurse mid-wife and many visiting specialists from La Crosse are able to care for all health needs from earaches to broken bones. Behavioral Health services are also available in the Clinic. The Gundersen Lutheran Eye Clinic - Prairie du Chien is located in downtown Prairie du Chien and provides vision services for all ages from routine exams to eye injuries.

The Franciscan Skemp Healthcare – Prairie du Chien Clinic is located at 800 East Blackhawk Avenue. Clinic services are centered around four family medicine physicians and their excellent support staff, including full laboratory and radiology services.  All of our physicians also provide complete obstetrical care.  Additionally, specialty physicians and other medical professionals from the Franciscan Skemp La Crosse Campus are in Prairie du Chien on a regular basis.  If you need more specialized care, our healthcare providers can refer you to one of our many specialists at the Franciscan Skemp La Crosse Campus.  The Franciscan Skemp Prairie du Chien Clinic also offers Behavioral Health Services through an on-site clinical therapist who works with children, adolescents, adults and couples.  If necessary, medical staff also provides care if you are admitted to Prairie du Chien Memorial Hospital.

Great River Community Medical Clinic is a new family practice clinic located at 100 N Marquette Road Suite 106.

Prairie Maison Nursing Home, located at 700 South Fremont Street, provides long-term care services to the community by means of skilled nursing care, intermediate care, hospice care, physical, occupational, and speech therapy.  Prairie Maison is owned and operated by Community Health Services corporation.

Prairie Memorial Housing/Bluff Haven  is owned and operated by Community Health Services Corporation, a nonprofit healthcare provider governed by a 12 member Community Board of Directors.  Other facilities in the area that are owned by Community Health Services include LaBatisse on Dousman, 24 Senior Apartments in Prairie du Chien, and Sannes Skogdalen Heim, a 66 bed skilled nursing facility in Soldiers Grove.  Bluff Haven is now open at 700 South Fremont Street, Prairie du Chien.  Other facilities in the new complex include the 6 unit Bluff Haven Memory Care facility for individuals with Alzheimer's and/or memory loss; Residence at Bluff Haven, a 24 unit senior apartment complex; and  Village Plaza, where the public will be welcome to enjoy our library, "Books & Beyond";  "Brown's Bistro" cafe; "Cuts & Curls" hair salon; "The Commons" community room, and indoor and outdoor patio spaces.  The services provided at Bluff Haven will include meals, housekeeping, laundry, activities, assistance with bathing, toileting, dressing, and much more.  Bluff Haven is the first phase of the Planned Community Development; the second phase shall include Cottages, Duplexes, and/or Condominiums.   608-326-8472.
